Facial bone implants can be used on their own to augment and provide structure to the face and facial skeleton, improve symmetry, and give a well-defined jawline and cheek region. Or, they can be used together with orthognathic jaw surgery. The most powerful correction often entails the use of both jaw and facial bone implants together with orthognathic double jaw surgery. The decision to perform these at the same time as jaw surgery or in a separate staged fashion will be considered and discussed, specific to your unique case and facial bone positions, and planned procedures. As mentioned above, an osseous genioplasty and bilateral jaw angle implants can be performed in concert, giving a wrap-around effect, with a combination of more natural autologous with alloplastic (implant-based) manipulation. Â

I wanted to talk a little bit about some of the facial bone implants that we do and the types of materials um in the common sites that we use these. So facial bone implants uh are basically pieces of structure that are constructed and they can be either custom custom to your face and your CT scan that we design digitally and have them 3D printed or 3D build so that they fit perfectly in some of the areas or they can be offtheshelf or stock meaning um similar to other types of implants for instance in orthopedics all of the hip and knee replacements are typically stock of different sizes. is for breast implants. These are typically stock of different sizes and volumes. This is true with facial bone implants too where they can be small, medium, and large. Uh and different designs to fit certain or different facial bones. The frequent places that we’re using these include the cheekbone and the bone underneath the lower eyelid. So to support this area to support the lid to give your cheeks uh more balance or prominence there are areas underneath your nose to called the puriform area or the perar or the midface uh region that sometimes we’ll use implants. We can sometimes use it in the chin especially for looking for projection of the chin or vertical length of the chin and we frequently use them along the jawline um including the jaw angle. Uh, and we can have something called a wraparound implant. That means there’s an implant on the jaw angle, the lower border of the jaw that meets with a chin implant and then continues to the opposite side. Typically, those would certainly be custom so that it fits in a bespoke manner with your jaw and your jawbone. The materials we’re typically using are something called peak uh or PEC, which are these very uh high density plastics, poly ether, etheral ketone. They can be 3D printed with one of various companies. They have areas within them that enable screw access and to be fixated on these various bones with screws. We also use something called porous polyethylene which is a slightly softer plastic that has openings or pores. The advantage of these pores is that it allows some of our um blood cells or macroofagages that fight infection to make their way in there and to fight infection and maybe reduce the infection rate. a little bit. There’s also good ingrowth uh into these implants. The downside of that is if ever these need to be removed, they can be more difficult uh to be removed. So, it’s sort of a double-edged sword where we want some more ingrowth, but if ever we need to remove them, they can be more difficult. The last area that I typically use is silicone, which again can be custom or stock. These are a little bit softer. They work well in older patients and old older facial aesthetic facelift women particularly along the cheeks or on the uh infraorbbit region. Less often do I use these on the jawline and chin. Uh but these are the three options. Uh frequently we’ll do facial bone implants on their own. augmentation. You know, this can create a really excellent aesthetic balance, but frequently we’ll do them in concert with other procedures, too, such as faceelift and facial rejuvenation procedures or such as orthogonathic jaw surgery. Um, and they can be really nice adjuncts to all of these uh procedures alone in isolation versus uh in concert with facial rejuvenation or orthognathic jaw surgery. So, thanks for listening. Facial bone implants, they can be done in isolation, but they’re also really excellent adjuncts to be done during jaw surgery. So, we know that repositioning the jaws can have a very powerful effect on the aesthetics in the tooth position and the balance of where the upper jaw bone is, the lower jaw bone is, the projection, tightening the chin throat distance and angle, and especially during this with genioplasty concurrently. However, in some patients, we know that there’s asymmetry from one side to the other or the jaw bone is underdeveloped and creating a nice angular component or angular definition and a shadow along the jawline is preferred and is the aesthetic that we’re looking for. So, these can be really nice adjuncts to doing that and placing that at the time of jaw surgery. So, when we do this, we’ll typically have a 3D plan for the orthognathic surgery and in that plan, we can look at and manipulate the bone itself and understand where the bone will end up and then we want to plan our implants based on that final orthognathic position. We don’t want to plan that based on the pre-orthognathic position or pre-op position, but we want to do it post-operatively. The post-op position, but do it at the time of jaw surgery. And common places that we’ll do it is the inferior border of the jaw, jaw angle, as well as the cheeks, the malar, the infraorbits. And this can be done really nicely concurrent with jaw surgery. So people ask about is it one versus the other. Is it facial bone implants or jaw surgery or can you know are they mutually exclusive? But they’re they’re not mutually exclusive. There are some situations where someone might be a candidate for facial bone implants alone. And in those situations typically there’s no airway issue, there’s no TMJ issue. the basil component in the structure of the upper lower jaw are quite well positioned and there may be some flattening of the cheekbones or some lack of definition along the jawbone and in those cases facial bone implants alone may be indicated. However, we should look and make sure in our history we’re trying to elicit if there’s any airway issues. Uh if there’s other things that can be achieved by doing orthognathic surgery, too. Because when we do orthogonathic surgery, this has a profound functional benefit as well. Because when we’re advancing the jaws, it’s helping open up the airway. It’s giving our tongue more space, which can help with speech and the feeling of tongue crowded uh being crowded. uh and it can really reposition our own natural bone and at times we still want to do facial bone implants on top of that as a nice adjunct. So facial bone implants can be done in isolation. Orthogonathic surgery can be done in isolation uh but they’re not mutually exclusive and it’s not an eitheror type situation. We want to evaluate every patient and see what’s best in terms of the functional and aesthetic goals. So thanks for listening.
